Part-Time In-Home Physician - CVS Health

Signify Health, part of CVS Health®, is seeking a Part-Time Physician (MD or DO) to conduct In-Home Health Evaluations in Grafton County and Lebanon, NH, engaging with health plan members in the comfort of their homes to bridge gaps in care.

In this role, you will bring comprehensive, personalized health evaluations directly to people's front doors. Visits last approximately 45 minutes and allow for unhurried, one-on-one connections. Evaluations include medication and medical history reviews, physical assessments, and optional plan-ordered diagnostic tests (e.g., diabetic eye exams, spirometry, bone density screenings).

Using an iPad with an intuitive clinical workflow, you will document health findings. You are not required to prescribe medicine, order lab tests, or alter members' current treatment regimens.

Key Responsibilities

• In-Home Health Evaluations: Perform comprehensive 45-minute in-home health evaluations and diagnostic/preventive screenings for health plan members across Grafton County.
• Member Engagement: Build trusted relationships with individuals, reviewing medical histories, medications, and overall quality of life in their home environments.
• Diagnostic & Preventive Services: Provide diagnostic services as ordered by health plans (spirometry, diabetic eye exams, bone density screenings, etc.).
• Virtual Evaluations: Conduct virtual visits as needed to support care delivery.
• Care Coordination: Identify environmental risks (e.g., home mobility hazards) and make referrals to health plans and primary care physicians (PCPs) for ongoing care.
• Travel: Travel locally across primary coverage areas (Lebanon / Grafton County) and within licensed state coverage areas as required.

Work Environment Notice

As an in-home clinician, your primary workplace is the private residence of health plan members. Work environments vary widely. Candidates must be able to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, airborne allergens, or irritants.

Requirements

Qualifications & Credentials

• Active, unrestricted MD or DO license(s) in New Hampshire (or coverage areas).
• Multi-state medical licenses are preferred.
• Ability to cover primary locations in Lebanon and Grafton County, NH.

• Compensation Structure: Hourly base rate ($105.00/hr) plus additional incentive pay per completed In-Home Health Evaluation and ordered Diagnostic & Preventive Service (DPS).
